A 25-year-old woman presents with a twelve-month history of a generalized itchy rash. A light scratch resulted in the appearance shown.

What is the diagnosis?

Dermographism.

What conditions are associated with it?

Idiopathic urticaria and other forms of physical urticaria sometimes coexist with typical dermographic urticaria. There is an increased incidence of autoimmune thyroid disease in those with idiopathic urticaria and/or dermographism.

What is the treatment?

Oral non-sedating antihistamines, taken as required, usually reduce wealing and irritation. Erythema may persist. The patient should keep cool, and avoid scratching. Phototherapy (UVB) can be helpful.
