Accurate diagnosis of lesions on the ear requires a careful history, thorough skin examination in a good light, and sometimes a biopsy. Inflammatory and neoplastic conditions may appear similar, often presenting as scaly or ulcerated plaques or nodules.

Squamous cell carcinoma in-situ
Outline the clinical features
A solitary indolent scaly plaque may be squamous cell carcinoma (SCC) in-situ (Bowen's disease). Larger than solar keratosis, it grows slowly, often reaching several centimetres in diameter before diagnosis. The border is often indistinct and irregular. Bleeding and crusting are frequent.
What are the treatment options?
Options for management include firm cryotherapy, 5-fluorouracil cream, imiquimod cream, photodynamic therapy, shave, curette and electrosurgery or excision biopsy.
SCC in-situ recurs in 20-30% of cases irrespective of which treatment is selected. The same treatment may be repeated or another selected.
