Allergic contact dermatitis to Lanacaine

The image shows an acute dermatitis affecting the vault of the axilla. In this case, the dermatitis followed the application of Lanacaine (benzocaine); patch testing confirmed hypersensitivity to topical anaesthetic agents (caine mix positive). Other allergens causing contact allergic dermatitis in the axillae include fragrances and preservatives in underarm deodorants.
Irritant contact dermatitis is much more common than allergic contact dermatitis; likely irritants include aluminium salts in antiperspirants, perspiration, tight clothing and woollen garments.
Potent topical steroids are warranted for an acute dermatitis for a few days, but if on-going treatment is required, hydrocortisone cream is safest.
