What is the diagnosis?
Cutaneous polyarteritis nodosa
What are the clinical features of this condition?
The image shows considerable scarring around a clean shallow ulcer on the ankle. The clue to the diagnosis is the angulated livedo pattern vasculature on her foot, indicative of a vasculitis affecting medium sized arteries.
Cutaneous polyarteritis nodosa results in malaise, arthralgias, myalgias, arterial nodules, livedo reticularis and chronic leg ulceration. It may be associated with a small vessel vasculitis, which causes episodes of palpable purpura.
