What is the diagnosis?
Tinea manuum. This was a surprise to the clinician, as contact dermatitis was thought more likely!
What clinical features distinguish this from other hand rashes?
Tinea manuum is nearly always unilateral or at least markedly asymmetrical. It may slowly spread from a single focus on the palm to the dorsum of the hand (or vice versa). The palmar lines tend to appear more prominent. Chronic tinea manuum results in mildly irritable dryness and fissuring and is nearly always associated with bilateral tinea pedis. Fungal infection may be present in one or more nails.
What diagnostic tests are available?
The most important diagnostic test is skin scrapings for mycology. Trichophyton rubrum was cultured from hands and feet in this case.
What treatment should be recommended?
Confirmed dermatophyte infection should be treated with topical and if necessary oral antifungal agents. The duration of treatment may be up to three months or longer if onychomycosis is present.
