Make a diagnosis.
Positive patch test to paraphenylenediamine.
What is the patient's likely occupation?
Hairdresser.
Paraphenylenediamine is the principal allergen in hair dye and is commonly responsible for acute and chronic occupational dermatitis in hairdressers. More than 50 compounds used in hair dyes have recently been banned in European Union countries because of toxic or allergic potential. Hairdressers have a very high risk of hand dermatitis because of exposure to irritants (water, detergents, other chemicals and hair shafts) and allergens (metals, fragrances, preservatives, hair dye, perming solution, bleaches). Patch testing is useful to sort out which potential allergen/s is/are responsible.
