Make a diagnosis.
Chronic hyperkeratotic palmar eczema due to contact dermatitis to cement; potassium dichromate allergy.
What is the patient's likely occupation?
Builder.
Potassium dichromate is a common allergen identified by patch testing. A positive response is most often observed in builders or construction workers, as the allergen is present in cement. Wet cement is also responsible for alkali chemical burns and irritant contact dermatitis. However, chrome allergy may induce chronic hand dermatitis long after contact with cement has ceased; characteristically a psoriasiform hyperkeratotic palmar eruption, as in the case illustrated. Involvement of the feet may relate to footwear, as chrome is a component of the leather tanning process.
