Phytodermatitis (contact dermatitis from plant).
What is the patient's likely occupation?
Gardener.
Plant dermatitis is characterised by an acute dermatitis on areas in contact with the plant, most often forearms and lower legs. In most cases, it takes the form of linear erythematous plaques, bullae and swelling in an irregular distribution. Airborne pollen may cause more diffuse dermatitis, often affecting face and neck. Mechanisms include irritants (a prickly branch or leaf), allergy (most often involving toxicodendron or rhus tree) and photocontact reactions (involving psoralens in weeds, vegetables, and fruit). Irritants and allergens are often distinguished by knowledge of the causative material but may require patch testing.
