This 80-year-old man has had itchy papular lesions on his limbs and trunk for 4 months. He has long-standing type 2 diabetes mellitus.
There are papules with a central slough.
Renal disease. Reactive perforating dermatosis is strongly associated with both diabetes mellitus and chronic renal disease, the latter of which can be caused by diabetes.
Transepithelial elimination of altered dermal collagen.
Topical occluded and intralesional steroids, sedating antihistamines, phototherapy (both UVB and PUVA).
